Our Supported Living Homes in Oxfordshire offer a unique opportunity to make a difference in the lives of our service users while advancing your career. As a Supported Housing Coordinator, you will be responsible for coordinating the provision of support services, promoting independence, and enhancing the quality of life for our service users.

Key Responsibilities:
* Coordinate the delivery of support services, ensuring seamless transitions and achieving individual goals.* Develop and maintain strong relationships with stakeholders, including families, healthcare professionals, and local authorities.* Participate in on-call duties to provide emergency support, including evenings and weekends.* Collaborate with colleagues to ensure effective communication and teamwork.* Monitor and evaluate the quality of our services, identifying areas for improvement and implementing changes as needed.

Requirements:
* A QCF in Health & Social Care Level 5 Diploma (or equivalent).* A valid full UK manual driving license and ability to travel independently as required.* Experience in a similar setting or role.* Strong communication and interpersonal skills.* Ability to work collaboratively as part of a team.* Strong knowledge of principles of learning disabilities, autism, relevant legislation, and CQC standards.
