We are looking for a skilled Social Worker to lead our Hospital Discharge team at Northumbria Specialist Emergency Care Hospital. As a Hospital Discharge Social Work Lead, you will oversee the assessment and planning of safe hospital discharges, ensuring seamless transitions for patients and their families.

This role requires excellent interpersonal and communication skills, as well as a clear understanding of the Care Act and Local Authorities' statutory duties. You will work collaboratively with internal and external teams to identify and address complex needs, prioritizing patient-centered care.

Key responsibilities include developing and implementing care plans, conducting assessments, and providing support to patients and their loved ones. A competitive salary range of £38,626 - £42,708, plus benefits such as 26 days annual leave (rising to 31 days after 5 years) and automatic pension enrollment make this an attractive opportunity.
