Key Responsibilities

This is an exciting opportunity to join our team as a CNA-Home Care professional. As a member of our Discharge Lounge team, you will be responsible for providing high-standard care to patients suffering from acute medical illnesses.

You will work closely with the multidisciplinary team to provide holistic care to patients, including those transitioning to home or community-based care.

To be successful in this role, you will need to possess excellent communication skills, a compassionate and caring attitude, and the ability to deliver high standards of care.

You should also be willing to work flexible hours, including weekends and night shifts.
