Job Description

This is an exciting opportunity to join our team as a CNA-Home Care professional. As a member of our Discharge Lounge team, you will be responsible for providing high-standard care to patients suffering from acute medical illnesses.

You will work closely with the multidisciplinary team to provide holistic care to patients, including those transitioning to home or community-based care.
