Are you a dedicated social worker seeking a challenging opportunity in hospital discharge? We have an exciting role for a Social Worker to join our Home Safe Hospital Discharge Service at Northumbria Specialist Emergency Care Hospital.
The ideal candidate will work closely with individuals, families, and carers to assess complex psychosocial needs, develop care plans, and deliver interventions to facilitate safe hospital discharges. This includes completing Urgent Needs Assessments, Mental Capacity Assessments, and collaborating with external services to access required resources.
A key aspect of this role is understanding the Care Act and Local Authorities' statutory duties. Our commitment to protecting and promoting vulnerable adults' welfare is paramount, and we expect all staff to share this commitment.
We offer a competitive salary range of £38,626 - £42,708, 26 days annual leave (rising to 31 days after 5 years), automatic enrolment into the Local Government Pension Scheme, and flexi scheme benefits.
