An exciting opportunity has arisen for a highly motivated individual to join our Community Nursing Team in the North East locality, providing planned care to patients in the community.
We are seeking a caring, motivated, and friendly Community Nurse to join our team, who possesses a commitment to interdisciplinary team working and innovative delivery of excellent standards of care.
Main duties of the job
You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for ongoing professional development. We actively encourage newly qualified nurses to join our team, offering a preceptorship programme and support for professional development in a nurturing environment.
The team provides nursing care to patients within their own homes or chosen place of residence, offering an alternative to hospital admission, supporting timely discharge, and enabling people to realise their potential.
About us
Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford, and Wrekin, and some services in surrounding areas too. These range from district nursing, health visiting, and running four community hospitals through to providing very specialist community care through talented and dedicated staff.
We are committed to clinical support, CPD & training, and patients, carers, and the public play a vital role in helping us to develop and improve our services.
Person Specification
Qualifications
- RGN NMC Registered.
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
- Experience of working in a multi-disciplinary / multi-agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft Office.
- Previous community experience.
Skills
- I T skills or willingness to undertake RIO Training.
- Up to date with Mandatory Training.
- Awareness of changing healthcare factors influencing change.
