Main Responsibilities
  • Deliver high-quality nursing care to patients in their own homes or chosen place of residence.
  • Work collaboratively with interdisciplinary teams to provide comprehensive care to patients.
  • Develop and maintain effective relationships with patients, carers, and other healthcare professionals.
  • Contribute to the development and implementation of care plans that meet the individual needs of patients.

  • Previous community experience.
