About the Role

We are seeking a highly motivated and caring Community Health Nurse to join our team in the North East locality. As a Community Health Nurse, you will be responsible for providing planned care to patients in their own homes or chosen place of residence.

Main Responsibilities
  • Deliver high-quality nursing care to patients in the community, promoting independence and enabling them to reach their full potential.
  • Work collaboratively with interdisciplinary teams to provide comprehensive care and support to patients and their families.
  • Develop and maintain strong relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
  • Stay up-to-date with changing healthcare factors and best practices to provide evidence-based care.
Requirements
  • RGN NMC Registered.
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
