Job Overview
Our Primary Care Mental Health Service is a pioneering initiative, recognized nationally for its innovative approach to delivering mental health care in primary care settings. Based in GP practices and neighborhood teams, we ensure that individuals with mental health needs receive timely, effective, and person-centered care.
We are seeking an enthusiastic and dedicated Primary Care Mental Health Practitioner to join our team. The ideal candidate will possess excellent communication skills, be able to conduct robust clinical assessments independently, and thrive in a dynamic and supportive environment.
Main Responsibilities
- Work collaboratively with Mental Health Liaison Practitioners to facilitate mental health assessments in primary care and support GPs in joint decision-making regarding secondary care referrals, signposting, and transition of service users from secondary mental health services back to primary care.
- Provide expert support, education, and problem-solving approaches for adults of working age, enabling them to manage mild to moderate mental health difficulties.
- Contribute to the development of management plans for individuals with mental health needs in primary care, working closely with GPs and other healthcare professionals.
Requirements
- Hold NMC or HCPC registration.
- Be able to travel independently around the county, meeting strict time deadlines, and hold a full UK driving license.
