About the Role

We are seeking an experienced and skilled Advanced Clinical Practitioner to join our team at Wentworth 1 Primary Care Network. As a key member of our network, you will play a vital role in delivering high-quality patient care and contributing to the development of our primary care services.

Key Responsibilities
  • Work autonomously and be accountable for your own professional actions, utilizing advanced clinical skills and expert knowledge to deliver holistic care to patients accessing our primary care service.
  • Assess undifferentiated patients with a range of acute, non-urgent, and chronic medical conditions, utilizing your expertise to formulate differential diagnoses and develop effective management plans.
  • Provide safe, evidence-based, and cost-effective patient care, promoting health and well-being through health education, screening, and therapeutic communication skills.
  • Work collaboratively with multidisciplinary teams within the practice and across the wider primary care network to promote integrated and seamless pathways of care.
  • Monitor the effectiveness of your clinical practice through quality assurance strategies, such as audit and peer review.
Requirements
  • Registered Adult Nurse, Paramedic, or Pharmacist with the relevant governing body.
  • Registered as an independent non-medical prescriber.
  • A minimum of 2 modules at Level 7, including assessment of clinical examination skills.
  • Experience of working in primary care, with a broad and sound knowledge of complex and common conditions in adults and children.
  • Consolidated independent prescribing experience and use of examination skills in practice.
Desirable Qualifications and Experience
  • MSc (Advanced Practice) or Mentorship qualification.
  • Experience of working in a GP practice and using autonomous physical examination and assessment skills.
