Job Description

Job Title: Advanced Practitioner - Primary Care Network

Job Summary:

We are seeking an experienced Advanced Practitioner to join our team at Wentworth 1 Primary Care Network. As an Advanced Practitioner, you will play a key role in providing high-quality patient care and contributing to the development of our primary care network.

Key Responsibilities:

  • Provide safe, effective, and evidence-based patient care, including prescribing medication and ordering diagnostic tests as needed.
  • Work collaboratively with a multidisciplinary team to develop and implement patient care plans, ensuring seamless pathways of care.
  • Conduct home visits, telephone/video consultations, and telephone triage to improve service delivery and patient experience.
  • Participate in quality assurance strategies, such as audit and peer review, to monitor and improve clinical practice.
  • Develop and maintain professional relationships with patients, families, and carers, providing empathetic and supportive care.
  • Stay up-to-date with relevant professional standards and guidelines, ensuring compliance with regulatory requirements.

Requirements:

  • Advanced Practitioner qualification and registration with the relevant governing body.
  • Experience in primary care, with a strong understanding of patient care and clinical systems.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Ability to work independently and make sound judgments in complex situations.
  • Strong problem-solving and conflict resolution skills, with a patient-centered approach.
