Patient Safety Investigation Lead

About the Role
We are seeking a highly skilled and experienced Patient Safety Investigation Lead to join our Patient Safety Team at the North East Ambulance Service NHS Foundation Trust.
As a key member of our team, you will be responsible for monitoring incidents, supporting lead investigators, and ensuring we uphold the highest standards of care.
Key Responsibilities
- Support the implementation of the National Patient Safety Incident Response Framework.
- Oversee and ensure compliance with the duty of candour, addressing any emerging safety themes.
- Drive rapid learning and improvement by working closely with service lines to ensure lessons from incidents are effectively shared across the organisation.
- Generate detailed reports and present your findings at key governance and safety forums, making your voice heard at the highest levels.

Requirements
- Significant experience in human factors/PSIRF.
- Educated to degree level in a relevant subject.
- Evidence of continuous professional development at the equivalent of postgraduate level.
- Teaching/mentorship qualification or demonstrable experience.
- Accredited Leadership programme or willingness to work towards.
Skills and Knowledge
- Knowledge of the Health Service and current national initiatives, particularly in relation to Clinical Governance and Patient Safety.
- Clear understanding of the issues prevailing around patient safety.
- Knowledge of LFPSE reporting requirements.
- Knowledge of CNST/NHSLA requirements.
- Knowledge of patient safety incident management and LFPSE reporting.
Experience
- Experience of contributing to CNST/NHSLA requirements.
- Experience of advising and liaising with staff at all levels, preferably in the NHS.
- Experience of writing policies within a NHS environment.

- Committed to the development of patient safety and understand clinical governance issues.
- Ability to prioritise and work proactively.
