Job Summary

We are seeking a highly skilled and experienced Clinical and Operational Lead to join our Intensive Support Service Team as a Team Leader. The successful candidate will be responsible for providing clinical and operational leadership to the team, ensuring the delivery of high-quality care and services to individuals with learning disabilities and complex needs.

Key Responsibilities
  • Provide clinical and operational leadership to the Intensive Support Service Team, ensuring the delivery of high-quality care and services.
  • Develop and implement clinical and operational strategies to improve patient outcomes and service delivery.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery.
  • Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
  • Supervise and mentor team members to ensure they have the necessary skills and knowledge to provide high-quality care.
  • Develop and maintain relationships with community partners and stakeholders to ensure effective service delivery.
  • Monitor and evaluate service delivery, identifying areas for improvement and implementing changes as needed.
  • Ensure compliance with regulatory requirements and Trust policies and procedures.
Requirements
  • Registered Nurse (Learning Disabilities) or Allied Health Professional with a degree or diploma in a relevant field.
  • Proven experience in clinical and operational leadership, with a focus on learning disabilities and complex needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams and community partners.
  • Strong analytical and problem-solving skills, with the ability to develop and implement effective solutions.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Commitment to lifelong learning and professional development, with a focus on staying up-to-date with best practices and evidence-based care.
