We are seeking an experienced and skilled professional to join our team as the Clinical and Operational Lead for the Intensive Support Service (ISS). The ISS is a specialized community care service that provides intensive support to individuals with learning disabilities and complex health needs.
Key Responsibilities- Provide clinical and operational leadership to the ISS team, ensuring the delivery of high-quality care and services to patients.
- Develop and implement effective strategies to improve patient outcomes, service efficiency, and staff performance.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Conduct comprehensive assessments and develop individualized care plans that meet the unique needs of patients.
- Supervise and mentor staff to ensure they have the necessary skills and knowledge to provide high-quality care.
- Monitor and evaluate service performance, identifying areas for improvement and implementing changes as needed.
- Develop and maintain relationships with stakeholders, including patients, families, and community partners.
- Stay up-to-date with best practices, policies, and procedures, ensuring compliance with regulatory requirements.
- Master's degree in Nursing or a related field.
- Current registration with the Nursing and Midwifery Council (NMC).
- Minimum 5 years of experience in a leadership role, preferably in a community care setting.
- Proven track record of improving patient outcomes and service efficiency.
- Excellent communication, leadership, and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
- Knowledge of relevant laws, regulations, and policies, including the Mental Health Act and the Mental Capacity Act.
