About the Role

We are seeking a highly motivated and skilled Diabetes Prevention Specialist to join our team at Thrive Tribe.

This is an exciting opportunity to make a real difference in people's lives, empowering them to take control of their health and wellbeing.

Key Responsibilities:
  • Deliver evidence-based interventions that focus on diet, nutrition, physical activity, and behaviour change.
  • Facilitate group intervention sessions to support clients in achieving their goals and creating a safe and positive environment.
  • Promote the Healthier You NHS Diabetes Prevention Programme internally and externally, and improve referral pathways between other services and partner organisations.
  • Engage in ongoing training relevant to the role, such as Behaviour Change Level 1 and 2.
  • Work towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures.
  • Collaborate with colleagues across all Thrive Tribe services to contribute to new projects and service developments.
