Job Title: Advanced Practitioner for Learning Disability Community Team

Location: Warrington/Halton area

About the Job:

We are seeking an experienced Advanced Practitioner to join our Learning Disability Community Team. The successful candidate will work alongside a well-established team to provide expert clinical practice to clients with complex needs.

Main Responsibilities:

  • Provide expert clinical practice to clients who pose significant challenges/risk to themselves or others.
  • Assume continued responsibility for caseload and clinically support interventions to clients with complex challenging behaviour.
  • Responsible for the assessment of need, care planning and care management of clients and carers.

Requirements:

  • Up-to-date professional registration (e.g., RMN, RNLD, OT, SW).
  • Experience of working with clients with learning disabilities/mental health issues.
  • Postgraduate qualification (masters or equivalent level of training through experience).

Salary: £46,148 - £52,809 per annum (dependent on experience)
