We are seeking an exceptional Advanced Nurse Practitioner to join our team as a key senior member of the Intensive Support Function of the Warrington and Halton Learning Disability Community Team.

About the Role

This is an exciting opportunity for an Advanced Practitioner to work alongside the established Community Learning Disability Team, developing, initiating, leading, and managing clinical assessment, formulation, and interventions, staff training, and special projects consistent with the aims of the Trust, service plan, and service operational policy.

Key Responsibilities
  • Providing expert clinical practice to clients who pose significant challenges or risk to themselves or others with associated problems such as mental health issues, complex communication needs, profound and multiple disabilities, long-term conditions, and forensic behaviour.
  • Assuming continued responsibility for caseload and clinically supporting interventions to clients with complex challenging behaviour and/or forensic behaviours who may be at risk of placement breakdown and/or hospital admission.
  • Taking the lead role in facilitating/co-ordinating access to primary and secondary health care services for people with learning disabilities, developing/influencing, and implementing changes at both local and trust-wide levels.
Essential Requirements
  • Postgraduate qualification i.e. masters or equivalent level of training through experience.
  • Evidence of post-basic education to degree level or evidence of performance to equivalent level.
  • Evidence of post-basic education in Positive Behavioural Support degree level.

Salary: £46,148 - £52,809 per annum.
