About the Role
We are looking for an experienced mental health professional to lead our neighbourhood teams and work closely with staff and partners to deliver high-quality care. The successful candidate will have a proven track record of experience and achievement in mental health assessment, care of adults and/or older people.
Main Responsibilities
- Ensure that mental health pathways are delivered within the neighbourhood teams and work closely with both neighbourhood team staff and the locality teams to ensure smooth communication and transitions within local services.
- Undertake assessments to determine patient need and collaborate with key partners and stakeholders including local authorities and independent/private sector providers to meet the individual needs of patients and their families/carers whilst working at all times to reduce risk.
- To ensure that adults and older people experience a timely, effective and person-centred approach to the delivery of integrated care.
- To manage and provide clinical supervision and leadership to a defined group of Integrated staff working within locality especially mental health practitioners.
- The post holder will support the development of skills in relation to Multi-disciplinary working, discharge planning/transfer and co-ordination of care in order to achieve a standardised approach and competency within the locality and contribute to the reduction of avoidable bed days.
- Work within the model of the NT (Neighbourhood teams), providing clinical leadership on the matters relating to mental health interventions.
- Clinically supervise mental health practitioners within the NT and ensure that there is excellent communication and planning for mental health interventions across the NT and ICT.
- To promote the understanding of common mental health conditions and develop skills and knowledge in the wider neighbourhood team and with key partners.
- Ensure systems are in place to receive and prioritise mental health referrals to the team and delegation to staff members with the level of skill and competence required to meet patient need.
- Facilitate the assessment and care planning/interventions for patients whose needs may frequently be complex and multi-factorial in nature, collaborating with the integrated team, Advanced Practitioners/medical colleagues, social services and other stakeholders involved in the transfer of care.
- Engage proactively with key stakeholders to identify patients who require supportive intervention and case management to prevent avoidable hospital admission and enable those individuals to remain in their own home environment.
- Negotiate the timely transfer home for those patients whose presentation does not require them to remain within or the acute/community hospital environment.
- Work in collaboration with the integrated neighbourhood team to provide information, prepare patients and their families/carers for changes in the patients condition and actively encourage and support decision making and choice for end-of-life care including the use of fast track and CHC processes.
- Prioritise and manage own workload to ensure responsive care/interventions by staff with the level of skill and competence to meet patient need and provide advice and support to team members regarding the care/management plan.
- Ensure own leadership style facilitates effective communication, collaboration and motivation of staff and partners to achieve effective transfer of care.
- To assess, receive and review data regarding patients who regularly attend/are admitted to acute care, liaise with the patients.
- Lead in the development of annual audit plans for the Mental Health component of the neighbourhood team.
- Work collaboratively with the Team and professional leads to support professional and practice developments including CPFT Clinical Supervision framework.
- Maintain confidentiality in accordance with the Data Protection Act.
- To work with the area mental health manager and peer group to develop and maintain countywide management systems to demonstrate effective delivery of mental health care.
- To ensure the delivery of mental health services within neighbourhood teams including reviewing available resources as required.
- Assist in the development and implementation of changes to modernisation and improve operational processes and delivery.
- Assist in leading person-centred, performance-focused approach to managing Integrated services.
- To be responsible for the management of and reporting on individual and team performance indicators and the development of remedial action plans as required.
- To undertake mandatory training as required by Trust policies Service.
Requirements
- Registered Mental Health Nurse or equivalent AHP or Social work qualification.
- NMC or HCPC registered.
- Demonstrate continuous professional development.
Salary and Benefits
We offer a competitive salary range of £40,000 - £60,000 per annum, depending on experience, plus a comprehensive benefits package, including pension scheme, life assurance, and annual leave entitlement.
