Job Summary:

We are currently seeking a Mental Health Support Worker - Young Adults to join our Community CAMHS team. This is an exciting opportunity to work with young adults with mental health needs, supporting them as they transition from children's services to adult services.

Key Responsibilities:

The successful candidate will be responsible for:

* Providing support and guidance to young adults with mental health needs

* Working closely with other professionals to ensure seamless transition to adult services

* Conducting risk assessments and developing personalized care plans

* Building relationships with young adults, their families, and other professionals to promote effective collaboration and support

Requirements:

The successful candidate will possess:

* A relevant qualification in mental health or a related field

* Experience working with young adults with mental health needs

* Excellent communication and interpersonal skills

* Ability to work effectively in a fast-paced environment
