Job Summary

The Wellbeing Coordinator will work closely with GP practices, community groups, and statutory services to promote health and wellbeing. You will be responsible for building relationships, providing emotional and practical support, and connecting people to community resources.

Key Responsibilities
  • Develop and maintain relationships with local community groups, VCSE organizations, and statutory services.
  • Provide emotional and practical support to individuals, helping them identify and address wider issues impacting their health and wellbeing.
  • Work with individuals to co-produce personalized support plans, promoting independence and self-management.
  • Support community groups and organizations to develop their capacity and sustainability.
Requirements
  • Degree in social work, mental health, or a related field.
  • Experience working in patient-facing roles, preferably in mental health or social care.
  • Advanced knowledge of Microsoft Office packages and data protection legislation.
  • Safeguarding training and experience.
  • Ability to network and build collaborative relationships.
