Job Responsibilities
  • Main Duties
    • Deliver healthy eating, physical activity, and health and wellbeing sessions to meet personalised care plans.
    • Work collaboratively with the Social Prescribing Link Workers, Care Coordinators, Pharmacists, and clinical teams within the PCN to connect patients to community-based activities.
    • Provide support to local community groups and work with other health, social care, and voluntary sector providers to support patients' holistic health and wellbeing.
    • Help establish and coordinate patient groups to aid self-management, peer support, and the creation of patient communities.
  • Key Skills
    • Able to communicate effectively and engage with individuals from the PCN, other agencies, including patients and the public.
    • Able to work across 2 networks and support the development of our primary care health and lifestyle coach service.

The salary for this role is estimated to be around £35,000 - £45,000 per annum, depending on experience. This figure is based on industry standards and comparable roles within the NHS.
