Personal Care Coordinator

We are looking for a skilled and compassionate Personal Care Coordinator to join our team at Belong Newcastle-Under-Lyme.
This is a part-time role with 4 positions available, each at 26 hours per week. As a Personal Care Coordinator, you will be responsible for coordinating the delivery of person-centred care to our residents, working closely with other staff members to ensure their physical, emotional, and social needs are met.
Your key responsibilities will include developing and implementing individualised care plans, collaborating with healthcare professionals to ensure best practice, and promoting independence and dignity through meaningful activities and conversations.
To be successful in this role, you will need to demonstrate strong organisational and communication skills, as well as a person-centred approach to caring for older people. You will also need to be able to work well under pressure and be flexible to meet the needs of the residents and the household.
