As a Disease Management Coordinator, you will play a vital role in supporting the Diabetes Specialist Team by managing patient records, scheduling appointments, and coordinating with healthcare professionals. In this role, you will have the opportunity to work closely with patients, relatives, and carers, providing accurate information and non-clinical advice as needed. You will also be responsible for maintaining accurate records, preparing reports, and analyzing data to inform decision-making. If you have experience working in an office environment, possess strong IT skills, and are comfortable with Microsoft Office and EMIS systems, we encourage you to apply.

Key Responsibilities:
• Provide administrative support to the Diabetes Specialist Team
• Manage patient records and maintain accurate data
• Schedule appointments and coordinate with healthcare professionals
• Maintain effective communication with patients, relatives, and carers
• Prepare reports and analyze data to inform decision-making

Requirements:
• 3 GCSEs grade C or above (or equivalent)
• NVQ Level 3 in administration or equivalent qualification
• Experience working in an office environment
• Strong IT skills and proficiency in Microsoft Office and EMIS systems
• Excellent communication and organizational skills

Working Arrangements:
• 15-hour permanent position based on the Furness General Hospital site
• Flexible approach to working hours, with some flexibility required to meet service needs
• Ability to travel to other sites as required

Salary:
£24,071 - £25,674 per annum (pro rata)
