NHS Gloucestershire Integrated Care Board - Children's Continuing Care Team

We are seeking a highly skilled and motivated paediatric nurse to join our Children's Continuing Care team. As a key member of the team, you will be responsible for undertaking individual Children's Continuing Care Assessments in line with the National Framework (2016).

The successful candidate will have specialist paediatric clinical knowledge and experience, with excellent communication and assessment skills. They will also have a good understanding of safeguarding and multi-agency working, as well as experience of working collaboratively in a multi-agency arena.

The role will involve working with providers of healthcare in the home/school as required, and informing the targeting of resources, monitoring, implementation and evaluation of Packages of Care and PHBs. The successful candidate will also be responsible for ensuring accurate and open communication and co-ordination with a range of organisations and individuals.

About the Role
  • Undertake individual Children's Continuing Care Assessments in line with the National Framework (2016).
  • Work with providers of healthcare in the home/school as required.
  • Inform the targeting of resources, monitoring, implementation and evaluation of Packages of Care and PHBs.
  • Ensure accurate and open communication and co-ordination with a range of organisations and individuals.
Requirements
  • Current NMC Registration in Children's Nursing.
  • Further training or significant experience in project management, financial management or supporting change management processes.
  • Experience and understanding of evaluating and measuring the performance of health services.
  • Comprehensive knowledge of project management and/or health information systems development.
  • Working knowledge of Microsoft Project ECDL.
  • A good understanding of the health and social care environment and roles and responsibilities within it.
  • Attention to detail combined with the ability to extract key messages from complex analysis.
  • Skills for managing projects ensuring they meet financial targets.
  • Clear communicator with excellent writing, report writing and presentation skills.
  • Skills for nurturing key relationships and maintaining networks.
  • Evidence of success in efficient and effective project and programme management.
