About the Opportunity

We are seeking an experienced Multidisciplinary Team Lead to join our Learning Disability and Autism Outreach Team at Devon Partnership NHS Trust.

This exciting new role involves leading a multidisciplinary team in the delivery of complex care packages to admission-vulnerable service users across Devon and for those discharged from The Brook.

As a Multidisciplinary Team Lead, you will be responsible for ensuring effective liaison with other partner agencies, families, and care providers at every stage of care.

You will lead a team of professionals in providing high-quality services that are safe and focused on people's recovery. We welcome applicants with a strong background in mental health, learning disability, or neurodiversity services.

Key Responsibilities

  • Lead a multidisciplinary team

  • Develop and implement care plans

Requirements

  • Degree in a relevant field (e.g., mental health, learning disability)
  • Experience in multidisciplinary team leadership
  • Strong communication and interpersonal skills
