Chronic Condition Care Coordinator

At Betsi Cadwaladr University Health Board (BCUHB) North Wales, we have a unique opportunity for a Chronic Condition Care Coordinator to join our team. This exciting role will see you working in the largest health organisation in Wales, covering a population of approximately 700,000.
As a key member of our Community hospital team, you will focus on delivering high-quality clinical care that addresses the specific needs of patients with chronic conditions.
You will be working closely with a Registered Nurse, adhering to the Health Board's established protocols, policies, procedures, pathways, and clinical governance requirements.
Key Responsibilities- Cultivating strong relationships with patients, carers, and multidisciplinary teams.
- Utilising your expertise to drive the delivery of exceptional healthcare services.
- Collaborating effectively with healthcare professionals to achieve shared goals.
- Maintaining accurate records and managing patient caseloads.
- Administering medications within predetermined guidelines.
