**About the Role:**

A new and innovative position has emerged within our Community hospital, offering a unique opportunity to work as part of a dedicated team. The successful candidate will deliver evidence-based high-quality clinical care, focusing on the direct needs of patients, including those with chronic conditions.

This role requires effective communication with patients, carers, multidisciplinary teams, and the wider health and social care community. You will use your personal effectiveness and knowledge to contribute to the development and delivery of a quality service, working proactively as a member of the healthcare team in support of registered practitioners.

You will manage a small defined caseload of patients under the supervision of the Ward Manager/Registered Nurse and/or Nurse Practitioner, administering prescribed medication within the scope of competence where designated according to NHS policy.

The estimated salary for this role is £35,000 - £45,000 per annum, depending on experience.
