Dementia Care Specialist

We are seeking an experienced Dementia Care Specialist to join our Enhanced Community Dementia team on a fixed-term contract for 10 months. The successful candidate will be part of a dynamic and growing service focused on providing specialist dementia care for pre, peri, and post-diagnostic patients in the Argyll and Bute area.
This is an exciting opportunity for a motivated and skilled professional to contribute to the development of our service, working in close collaboration with mental health nurses, occupational therapists, social workers, and other healthcare professionals. The Dementia Care Specialist will be responsible for facilitating rapid access to diagnosis, treatment, and support, while preventing avoidable hospital admissions and assisting with timely discharge.
The successful candidate will have well-developed clinical assessment skills, leadership abilities, and experience in compiling care plans in partnership with service users and carers. They will be able to work independently and as part of a team, demonstrating a commitment to excellence in dementia care.
The Enhanced Community Dementia service is an integral part of the Argyll and Bute Healthcare system, working closely with Community Health Services, Mental Health, and other agencies to provide comprehensive support to individuals with dementia and their families.

Key responsibilities include:
- Providing specialist dementia care and support to patients and their families
- Facilitating rapid access to diagnosis, treatment, and support
- Compiling care plans in partnership with service users and carers
- Working collaboratively with multidisciplinary teams to deliver high-quality care
- Contributing to the development of our service, identifying areas for improvement and implementing changes
The salary for this position is £34,000 - £40,000 per annum, depending on experience, and the working hours will be 37 hours per week. NHS Scotland is committed to reducing its full-time working week from 37.5 to 37 hours per week from 1 April 2024, with no change in pay. This reduction will also be applied pro rata for part-time staff.
