Job Summary: Integrated Care Practitioner

We are seeking an experienced Integrated Care Practitioner to join our team. The successful candidate will be responsible for providing a high-quality, patient-centred care service to patients with mild to moderate frailty and long term conditions.

About the Role

The Integrated Care Practitioner will work closely with the Integrated Care Team, Frailty MDT and local GP teams to provide proactive care services for patients with frailty or those with a long term condition identified as requiring an anticipatory approach.

The role involves developing effective care plans that meet the specific therapeutic needs of each patient, and ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.

The Integrated Care Practitioner will also be involved in championing primary care by working with local general practice and system partners in the provision of community-based healthcare services.

Main Duties of the Job
  • Work closely with the Integrated Care Team, Frailty MDT and local GP teams to provide proactive care services.
  • Develop effective care plans that meet the specific therapeutic needs of each patient.
  • Ensure appropriate pathways of care and communication via liaison and referral to other agencies as required.

**Salary:** £45,000 - £55,000 per annum
