    About the Role:

    As a Care Coordinator within our Adult Social Care Locality Teams, you will play a pivotal role in supporting individuals in Gloucestershire, understanding their needs, and connecting them to empowering resources that enable them to maintain control over their lives.

    About Us:

    Gloucestershire County Council is dedicated to fostering a supportive environment for our staff, ensuring they have the necessary tools and resources to excel in their roles. Our teams boast a collaborative management structure, comprising an Integrated Social Care Manager, Deputy Social Care Managers, Social Work Leads, and Social Care Leads. You will work alongside qualified Social Workers, Senior Social Workers, Adult Social Care Practitioners, and an administration team to achieve our goals.

    Key Responsibilities:

  • Providing exceptional support to individuals in Gloucestershire, working closely with them to understand their needs and develop personalized plans to empower them in maintaining control over their lives.
  • Collaborating with a multidisciplinary team, including Social Workers, Senior Social Workers, Adult Social Care Practitioners, and administration staff, to deliver high-quality services that meet the needs of our service users.
  • Working within a fast-paced environment, utilizing your passion for promoting people's strengths with a safe working approach to social work practice.

  • Social work qualification and SWE registration: Essential
  • A right to work in the UK: Essential

    What We Offer:

The estimated salary range for this position is £36,648 - £38,223.
