About the Role


We are seeking a highly motivated and experienced Adult Social Care Practitioner to join our Gloucester Locality Team.


As an Adult Social Care Practitioner, you will be responsible for undertaking assessments and support plans with individuals who have physical disabilities or other complex needs.


You will work closely with families, carers, and other professionals, agencies, and organisations across Gloucestershire to promote independence and enable individuals to remain in their own homes.


The ideal candidate will have experience of working in a health and/or social care setting and knowledge of the Care Act, with the ability to apply it in practice.


About You


To be successful in this role, you will need to have a relevant qualification equivalent to NVQ Level 3 or equivalent health and social care experience.


You will also need to have training related to health and social care assessments, as well as experience of managing a caseload and working with people with a range of physical disabilities, medical, mental health difficulties, and learning difficulties.


Good interpersonal and communication skills, as well as good IT skills, are also essential for this role.
