In this dynamic position, you will work closely with our experienced team to promote independence, enable individuals to remain in their own homes, and reduce the need for hospital admission or long-term care.

Key responsibilities include:

  • Conducting thorough assessments and developing tailored support plans
  • Collaborating with families, carers, and other professionals to ensure seamless care delivery
  • Managing a caseload of individuals with complex needs, prioritising their well-being and safety

To succeed in this role, you must:

  • Hold a relevant qualification, such as NVQ Level 3, or equivalent Health and Social Care experience
  • Have training related to Health and Social Care assessments and management
  • Proven experience in managing a caseload of individuals with complex needs
  • Demonstrate excellent interpersonal skills, including verbal and written communication

We offer a competitive salary of £32,160 per annum, plus a range of benefits, including annual leave, pension scheme, regular supervision, training opportunities, and transfer options after one year in post.
