Community Mental Health Support Professional

This exciting opportunity is part of our Community Transformation programme, which aims to provide high-quality mental health services to individuals in Essex. As a Registered Mental Health Nurse, you will play a vital role in supporting service users and their families through our multidisciplinary team.
Key Duties and Responsibilities- Conduct thorough assessments to identify individual needs and develop tailored care plans
- Provide education on mental illness, medication, and adherence to treatment plans
- Collaborate with colleagues to ensure effective communication and coordination of care
- Minimum 12 months experience at Band 5
- Evidence of community exposure and post-qualification training
