Mental Health Service Coordinator

About the Role:

We are seeking a highly skilled and experienced Mental Health Service Coordinator to join our Intensive Outreach Team in South East Essex. As a key member of our team, you will work closely with service users who require intensive support and care. Your primary responsibility will be to assess, plan, deliver, and evaluate care for these individuals, ensuring that their needs are met and they receive the best possible outcomes.

Key Responsibilities:
  • Coordinating care for service users who require intensive support and care
  • Developing and implementing care plans that focus on empowerment and individual strengths
  • Working closely with service users and their carers to provide co-produced, personalized support
  • Assessing and managing risk associated with service users
Requirements:
  • Current professional registration with a relevant healthcare or social work body
  • Extensive experience in care coordination in a community mental health setting
  • Excellent communication and interpersonal skills
