Community Mental Health Care Coordinator

As a Community Mental Health Care Coordinator, you will play a vital part in coordinating care for individuals with complex mental health needs. You will work collaboratively with a multidisciplinary team to assess, plan, and deliver care that meets the unique needs of each service user.
