Advanced Mental Health Practitioner

About the Role:

We are seeking a highly skilled and experienced Advanced Mental Health Practitioner to join our Intensive Outreach Team in South East Essex. As a key member of our team, you will work closely with service users who require intensive support and care. Your primary responsibility will be to assess, plan, deliver, and evaluate care for these individuals, ensuring that their needs are met and they receive the best possible outcomes.

Key Responsibilities:
  • Assess and manage risk associated with service users
  • Provide co-produced, personalized support and care planning with service users and their carers
  • Develop and implement care plans that focus on empowerment and individual strengths
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Administer and monitor medication as required, liaising with medical staff as necessary
Requirements:
  • Current professional registration with a relevant healthcare or social work body
  • Extensive experience in care coordination in a community mental health setting
  • Excellent communication and interpersonal skills
