About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our Intensive Outreach Team in South East Essex. As a key member of our team, you will be responsible for managing a caseload of service users who require complex care and support.

Main Responsibilities

  • To assess, plan, deliver, and evaluate care for service users referred to the Intensive Outreach Team, including risk assessment and management.
  • To manage a caseload of patients as named key worker/care coordinator/lead clinician, working within a psychosocial/family-centred framework.
  • To organise and plan an active clinical caseload, liaising closely with other agencies and disciplines.
  • To undertake evaluation of clinical symptoms and disabilities, advising medical staff on necessary interventions.
  • To coordinate CPA care plans for service users with complex mental health problems, promoting engagement and intensive packages of care.
  • To work within new models of patient care, designated by the trust, ensuring seamless client care.
  • To provide formal clinical supervision to students and staff, receiving supervision from relevant line managers or senior practitioners.

Person Specification

  • Current professional registration with appropriate body in healthcare or social work.
  • Extensive experience in care coordinating people in community mental health settings.
