Care Coordinator for Adults with Disabilities

About the RoleWe are seeking a Senior Support Worker to join our team at Oak House and Acorns, a small community home in Worsborough, Barnsley. As a Senior Support Worker, you will be responsible for auditing and administering medications, updating and assessing care plans, supervising a small team of social care staff, and liaising with families and health professionals.
Key Responsibilities- Auditing and administering medications to ensure the best possible outcomes for our service users;
- Updating and assessing care plans to reflect individualised needs and goals;
- Supervising a small team of social care staff to ensure they have the necessary skills and support to deliver high-quality care;
- Liaising with families and health professionals to ensure seamless communication and coordination of care.
- A competitive salary of £13.54 per hour, based on a 37.5-hour week, reflecting the importance of this role in supporting our service users;
