Care Manager for Adults with Learning Disabilities and Autism

We are seeking an experienced Care Manager to lead a Supported Living service in Barnsley, South Yorkshire, providing support to adults with learning disabilities and autism.
About the Role:- Lead a team of care professionals in delivering person-centered support plans that enable individuals to achieve their goals.
- Ensure effective health and wellbeing outcomes for people supported, collaborating with relevant professionals as necessary.
- Contribute to business development objectives, supporting the growth and success of the organization.
- Maintain local market knowledge, identifying opportunities to maximize benefits for individuals and the organization.
- Level 3 Social Care qualification or equivalent.
- Willingness to work towards NVQ Level 4/5 in Health and Social Care.
- Experience in a supervisory role within care or support settings.
- Strong understanding of CQC regulations and principles.
- Ability to work flexibly, including shift work and on-call duties.
