About the Role

We are seeking an experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare. As a key member of our team, you will play a vital role in the continuous delivery of care to our service users.

Key Responsibilities:

  • Manage a caseload of care packages and coordinate care delivery to meet the needs of our service users.
  • Apply your communication and relationship-building skills to effectively manage rotas for your care teams and respond to day-to-day changes.
  • Act as the first point of contact for service users, their families, and other healthcare professionals.
  • Work collaboratively with our team to ensure the highest standards of care are delivered.
  • Support the out-of-hours on-call service on a rotational basis.

Requirements:

  • Experience in care coordination and management.
  • Strong communication and interpersonal skills.
  • Ability to work well under pressure and think on your feet.
  • Valid driving license and access to a vehicle.
  • Commitment to supporting the out-of-hours on-call service.

What We Offer:

  • A competitive salary between £29,460 - £35,460 per annum.
  • Payment for on-call duties.
  • Opportunity to earn a bonus of up to £2,000 per annum.
