About the Role

We are seeking a skilled Care Coordinator to join our dedicated team at Apollo Home Healthcare. As a key member of our team, you will play a vital role in the delivery of high-quality care to our service users.

Key Responsibilities:

  • Manage a caseload of care packages, ensuring seamless delivery of care services.
  • Apply effective communication and relationship-building skills to manage rotas for care teams, respond to day-to-day changes, and act as the first point of contact for service users, their families, and healthcare professionals.
  • Utilize adaptability and initiative to work well under pressure, making sound decisions and taking proactive steps to resolve issues.
  • Collaborate with healthcare professionals to ensure comprehensive care delivery, prioritizing the needs of service users.
  • Support the out-of-hours on-call service, working on a rotational basis to ensure 24/7 coverage.

Requirements:

  • Experience in care coordination, with a strong understanding of care delivery principles.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and healthcare professionals.
  • Ability to work well under pressure, prioritizing tasks and managing multiple responsibilities.
  • Valid driving license and access to a vehicle, with a willingness to travel across the region as required.
