- Support the out-of-hours on-call service, working on a rotational basis to ensure 24/7 coverage.
Requirements:
- Experience in care coordination, with a strong understanding of care delivery principles.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and healthcare professionals.
- Ability to work well under pressure, prioritizing tasks and managing multiple responsibilities.
- Valid driving license and access to a vehicle, with a willingness to travel across the region as required.
