About the Role:

We are seeking a skilled Care Coordinator to join our dedicated team in St Helens, Merseyside. As a Care Coordinator, you will play a key role in the continuous delivery of care to service users.

Key Responsibilities:

  • Manage a caseload of care packages and understand the varied nature and fast-pace of the role.
  • Apply your gained communication and relationship building skills to effectively manage rota's for your care teams.
  • Respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.

Requirements:

  • Experience in care coordination and a strong understanding of care delivery.
  • Excellent communication and relationship building skills.
  • Ability to work well under pressure and use your initiative.
  • Valid driving licence and access to a vehicle.

  • Competitive salary between £20,460 - £35,460 per annum.

Working Hours:

This is an office-based role, Monday to Friday, 9am – 5pm. At times, travel across the Merseyside region will be required.

On-Call Service:

You must be committed to supporting the out of hours on call service, which is covered on a 1 weeknight per week and 1 weekend in 4 rotational basis across the team.
