We are seeking an experienced Coordinated Care Delivery Specialist to join our dedicated team in St Helens, Merseyside. This is an office-based role, Monday to Friday, 9am – 5pm but at times, travel across the Merseyside region will be required.

The successful candidate will have solid planning, coordination, and problem-solving skills, with a strong background in managing caseloads of care packages. They will apply their communication and relationship building skills to effectively manage rota's for their care teams, respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.

This is a challenging yet rewarding role that requires adaptability and the ability to work well under pressure. The successful candidate will have a proven track record of using their initiative and thinking on their feet to deliver high-quality care services.

We offer a competitive salary between £23,791 - £39,649 per annum, which includes payment for on-call. There is also the opportunity to earn bonus of up to £2,500pa.

A driving licence and access to a vehicle are essential requirements for this role, as you will need to travel across the Merseyside region. You must also be committed to supporting the out of hours on call service, which is covered on a rotational basis.
