Care Coordinator Specialist

About the Role
Apollo Home Healthcare is seeking an experienced Care Coordinator to join our dedicated team in a key role that requires strong planning, coordination, and problem-solving skills. As a skilled Care Coordinator, you will be responsible for managing a caseload of care packages and understanding the varied nature and fast-pace of the role, along with all elements of associated care delivery.
Key Responsibilities
- Manage rota's for care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
- Apply communication and relationship building skills to effectively manage care delivery, ensuring seamless coordination and high-quality care.
- Work collaboratively with care teams to ensure efficient care delivery, adapt to changing circumstances, and utilize initiative and thinking on your feet.
- Support the out-of-hours on-call service, which is covered on a rotational basis across the team.
Requirements
- Hold a Driving Licence and have access to a vehicle for travel across the region.
- Be committed to supporting the out-of-hours on-call service.
- Have a strong understanding of care delivery, coordination, and management principles.
What We Offer
- A competitive salary between £29,460 - £35,460 per annum, including payment for on-call.
- The opportunity to earn a bonus of up to £2,000 per annum.
