As an adept Care Management Specialist, you will be accustomed to overseeing a diverse portfolio of care services and appreciate the dynamic and fast-paced nature of the position, along with all aspects of related care delivery.

Your developed communication and relationship-building skills will be essential in effectively managing schedules for your care teams, promptly addressing any daily changes, and serving as the primary contact for clients, their families, and other healthcare professionals.

Regardless of the care environment in which you have gained your Care Management experience, you will recognize that each day presents unique challenges, demonstrating your flexibility and ability to perform well under pressure, utilizing your initiative and quick thinking.

Therefore, if you possess the expertise we seek and share our passion for care, we would be delighted to hear from you.

This position is office-based, operating Monday to Friday, 9am – 5pm, with occasional travel required. Candidates must hold a valid Driving Licence and have access to a vehicle.

Additionally, you must be willing to support the out-of-hours on-call service, which is managed on a rotational basis.

We offer a competitive salary ranging from £29,460 to £35,460 per annum, which includes compensation for on-call duties. There is also potential to earn a bonus of up to £2,000 annually.
