About the Role

Apollo Home Healthcare Ltd is seeking an experienced Care Coordinator to join our dedicated team in a key role that requires strong planning, coordination, and problem-solving skills.

Key Responsibilities

  • Manage a caseload of care packages and understand the varied nature and fast-pace of the role, along with all elements of associated care delivery.
  • Apply communication and relationship building skills to effectively manage rota's for care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
  • Work well under pressure, using initiative and thinking on your feet to adapt to changing situations.
  • Support the out of hours on call service, which is covered on a rotational basis across the team.

Requirements

  • Hold a Driving Licence and have access to a vehicle.
  • Be committed to supporting the out of hours on call service.
  • Have a competitive salary between £29,460 - £35,460 per annum, which includes payment for on-call, and the opportunity to earn a bonus of up to £2,000pa.
