About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our dedicated team at Apollo Home Healthcare Ltd. As a Care Coordinator, you will play a key role in the delivery of care to our service users, working closely with our care teams to ensure the highest standard of care is provided.

Key Responsibilities
  • Manage a caseload of care packages, understanding the varied nature and fast-paced environment of the role.
  • Apply communication and relationship-building skills to effectively manage rotas for care teams, respond to day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
  • Work collaboratively with care teams to ensure seamless care delivery, adapting to changing circumstances and prioritizing tasks accordingly.
  • Support the out-of-hours on-call service, working on a rotational basis to ensure 24/7 coverage.
Requirements
  • Hold a valid driving license and have access to a vehicle for travel across the Merseyside region.
  • Be committed to supporting the out-of-hours on-call service, working on a rotational basis.
  • Have a competitive salary between £29,460 - £35,460 per annum, including payment for on-call, and the opportunity to earn a bonus of up to £2,000 per annum.
