Key Responsibilities:
- Manage a caseload of care packages, ensuring seamless delivery of care services.
- Apply effective communication and relationship-building skills to manage rotas for care teams, respond to day-to-day changes, and act as the first point of contact for service users, their families, and healthcare professionals.
- Utilize your adaptability and problem-solving skills to work well under pressure, using your initiative and thinking on your feet.
- Collaborate with the team to ensure the delivery of care services meets the highest standards.
Requirements:
- Proven experience as a Care Coordinator in a healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work well under pressure and adapt to changing situations.
- Valid driving license and access to a vehicle.
- Commitment to supporting the out-of-hours on-call service.
What We Offer:
- Competitive salary between £29,460 - £35,460 per annum, including payment for on-call.
- Bonus scheme of up to £2,000 per annum.
- 25 days annual leave, plus Bank Holidays.
