**Estimated Salary: £25,000 - £30,000 per annum**
The successful candidate will provide support to the physiotherapy and occupational therapy team in the planning and delivery of holistic interventions and self-management programs for patients with palliative diagnoses. This role involves assisting with evaluating progress against set outcome measures and collaborative patient goals.
Key Responsibilities:- To provide assistance in the planning and delivery of a range of holistic interventions and self-management programs to support rehabilitative care approaches for patients with palliative diagnoses.
- To assist with evaluating progress against set outcome measures and collaborative patient goals, ensuring optimal quality of evidence-based care.
- To maintain excellent working relationships within the multidisciplinary team, promoting effective communication and coordination of patient care.
- To monitor patient progress, notifying therapists immediately if there are changes observed or concerns regarding safety and wellbeing.
