About the Role
This full-time position involves working as part of our community services team at Compton Care. Your primary responsibility will be to provide clinical, hands-on care to patients in their own homes or other community settings. You will work collaboratively with other healthcare professionals to ensure seamless care delivery and contribute to the development of best practices within our organization.

Key Responsibilities
- Provide specialist palliative care to patients with complex and incurable conditions
- Assess patient needs and develop personalized care plans
- Deliver evidence-based interventions to improve quality of life
- Work collaboratively with multidisciplinary teams to ensure seamless care delivery
- Contribute to the development of best practices within the organization

Requirements
To be successful in this role, you will need to be a registered general nurse or paramedic with post-registration experience. You should hold a healthcare degree and/or have taken a health assessment course or be willing to undertake this training. Advanced communication skills are essential, and you should have experience working with patients with complex needs. A passion for delivering high-quality palliative care and a commitment to ongoing professional development are also essential.
